You can print an application for the latest position and fax it to Craig Davis 847-776-6455. If you have any questions, call Craig at 847-226-9082 or 888-304-4545.
The application is six pages long. Make sure you print all six. You will have to make adjustment when you print the application.
Go to File then to print preview,
The % or where you adjust the page size should be changed to 30%. Next, go to page set-up and set margins to:
Left 0.25 Right 0.25
Top 0.25 Bottom 0.5
After adjustments click ok, then print!
You can also adjust the margins top and bottom more if needed by using the margin adjusters on the page. Just make sure all of the print is visible in the set-up preview.
